Since a LOT of people seem to be having trouble with this, here is a good template for a ban appeal, and bad templates for a ban appeal:
Good ban appeal:
Notes:
This person answers all of the questions and makes his in game name one of the first things in his post.
This person admits he did something wrong, and doesn't try to blame it on someone else. If you don't know why you were banned, please just say you want to know why you were banned. Don't say something like, "i didnt do anything wrong! unban me!"
This person is willing to start following the rules.

Bad ban appeal:
This person puts his in game name as the last part of his post.

This person lacks knowledge of the English language. Extra note:You can use slang and abbreviations and ignore grammar every now and then, but please don't do it to the point where it is not readable like this.
This person believes that since he didn't know the rules, the fact that we banned him is stupid.
This person admits he was the offender and does not blame it on someone else.
Terrible ban appeal:

This person has all the criticisms of the bad ban appeal.

Please forgive me. I didn't know that destroying other people's homes was against the rules and I didn't even know what griefing was (this is the first multiplayer server I've been on). I promise I will never do anything against the rules again. I've even looked at the rules for the server, and I promise never to break them.
